Default Getting New Pan Card - 05-31-2008

If PAN Card & PAN Allotement Letter, which are issued by income tax department during 1998, are misplaced, how I get new PAN card for the above PAN ?

Is it necesary to enclose the above details in "Request for new PAN card /or/and change or correction in PAN data application"

You can fill the application provided by Income tax department and they will issue a new card to you

Yes it would be necessary to furnish those details and any other details pertaining to the lost documents. But in case you don't have that info it shouldn't be a problem. A visit to the IT office and a new form should do fine.
